COVID-19 Vaccine Scheduling

Phoebe is currently vaccinating patients in accordance to FDA, CDC, and State of Georgia Department of Public Health guidelines. Frequently asked questions regarding the vaccine can be found here. Those eligible to receive vaccination at this time include:

  • All Georgians age 5 or older can receive the Pfizer vaccine. All Georgians age 18 or older can receive the Moderna vaccine. Individuals 18 an older can schedule an appointment below.
  • For individuals under 18, a parent or legal guardian will need to call 229-312-MYMD to schedule an appointment in Albany and Americus. To schedule an appointment in Sylvester, please call 229-776-2965.
  • The FDA, CDC, and Georgia Department of Public Health has released a list of individuals eligible to receive a booster. To view a list of eligible individuals, please click here.

There are two ways to schedule your vaccination appointment – simply click the button below and fill out the form, or call 229-312-MYMD to schedule in Albany and Americus or 229-776-2965 to schedule in Sylvester.


Things to remember:

  • Vaccinations are available in Albany, Americus, Buena Vista, Camilla, Leesburg, and Sylvester. Appointments can be made at the location that is most convenient for you.
  • Bring your photo identification.
  • For minors under 18, a parent or legal guardian must bring and sign a consent form.
  • You will receive a text message when it is your turn to schedule an appointment.
  • Please respond to the text message as soon as possible as appointments will fill up fast.
  • If a vaccine is not available, you will be placed on a waitlist and contacted via text when vaccine becomes available.

Guidelines to follow if you qualify for a COVID-19 vaccine and want to schedule an appointment at Phoebe.

  • If you are not feeling well, wait until you feel better to schedule an appointment.
  • Do not arrive more than 15 minutes before your scheduled appointment time.
  • All vaccine recipients will be screened for COVID-19 symptoms prior to vaccination.
  • All vaccine recipients are required to double mask. They must wear a cloth mask over a surgical mask. If you are in need of a second mask, we will provide you with one or both. 
  • Only one person will be allowed to accompany a vaccine recipient into the check-in and vaccination areas and only if it is medically necessary.
  • If you cannot make your appointment, please call to reschedule.
  • Following the vaccination, all recipients will be required to remain in an observation area for 15 minutes to ensure they do not experience any adverse reactions.
