Home Health Monitoring or TeleHealth uses telecommunications equipment placed in patients’ homes by our nursing staff. The system measures the patient’s vital signs such as pulse ox, blood pressure, and weight every day. The TeleHealth device also reminds the patient to take medications and answer simple questions about their condition.
Telehealth allows a nurse to monitor the patient daily while giving the patient their independence. The patient answers questions on the TeleHealth device each morning and then the device automatically sends the data to the nurse in the office via phone connection. This program decrease readmits to the hospital and allows the physician to be notified quickly if a patient’s status changes.
